Forms & Fees

Athletic Fees 2016-17

With the continued growth of the sports programs at CCS, comes the increased cost associated with each sport.  Athletic fees for new sports will be announced as they are added to the program. Fees for 2017-2018 include a athletic jacket, a 5×7 team photo, and a fully catered sports banquet.  Please keep in mind that the total cost of the program is considerably higher than the individual sports fee.  The difference between the total cost per player and the 2016-2017 sports fee is made up through booster club sales and admission fees along with concession sales and fundraisers.

In order to participate in any sports programs at CCS, students will need to have specific forms on file. See below…

Required Forms

Physical-Waiver-Concussion Forms

While a current physical is not required to participate in camps/open gyms, a new one is required before the first practice for any Junior, Middle or High School sport they intend to play.  Please take them to your child’s doctor to be completed. Sports physicals are also available at area pharmacies with minute clinics, but still must be on the required forms.

Non-CCS Athletic Participation Application

Students not currently enrolled in Cherokee Christian High School that desire to participate in the athletic program may be granted permission by the administration on a case-by-case basis if all standards of eligibility are met.

Parent-Athlete Manual

Athletes and their parents should be familiar with the policies of the athletic department and understand the what is expected of them academically and athletically. Download the Parent-Athlete Manual and read thoroughly. The last page must be signed by both and returned to your coach prior to the start of the season.

Game Attendance Permission Form

The sports program is an asset to CCS and we want to make it possible for all spectators to be able to attend a game.  All middle school students who want to attend a game will be allowed to stay after school and go to the game unattended by an adult.

GrowthCare charges will apply up until the game time.  GrowthCare staff will check the student out and allow them to go into the game. A permission form is required for each game attended. Please complete and turn in to the front desk the day of the game your student plans to attend.

This privilege is only for the middle school student – no siblings.
